Ask what the percentage is based on.

The rate is only one part of an administrator-fee arrangement. The base, cap and separate charges can change the result.

Follow the arithmetic.

This demo subtracts an allowed amount from a billed charge, applies a percentage to the reduction, limits that percentage fee by an optional cap, then adds an optional separate flat fee. The flat fee is outside the percentage cap in this model.

Read the agreement alongside the result.

A contract may define savings or cap charges differently. Other charges, services and cost-sharing rules can also matter. The fictional example cannot establish market prevalence, fiduciary compliance or whether a particular contract is appropriate.

Ask for evidence you can check.

  • An itemized calculation tied to the executed agreement.
  • Definitions of the billed charge, reduction and exclusions.
  • All separate fees and the scope of any cap.
  • Reporting, audit rights and a comparison of total plan costs.
