Patient and provider resource

Understand a payment reduction or downcoding question.

Record what changed, what explanation was given and what remains uncertain.

Compare the records

  • Original and revised service level or code, when known.
  • The remittance, explanation of benefits or other written determination.
  • The stated reason for the change.
  • Requests for an explanation or review and any response.

Distinguish evidence from suspicion

A payment reduction does not by itself establish that AI or automated review was used. Record whether the notice identifies automated review, the plan confirmed it, or the method is unknown. Avoid presenting an assumption as a fact.

Providers follow a distinct process

Provider payment disputes depend on the agreement, plan and applicable process. The builder includes provider-specific evidence prompts, but does not initiate a provider dispute or substitute for a patient’s benefit appeal.
