Start with the decision and your plan.
Find the written reason, plan category and appeal instructions. An appeal seeks reconsideration of a benefit decision. A complaint can address a service or process concern. One does not automatically substitute for the other.
Match the resource to the plan.
- Texas-regulated private insurance: review TDI’s consumer guidance.
- Private self-funded employer plan: review the plan documents and Department of Labor guidance.
- Medicare: use Medicare’s distinct complaints and appeals information.
- Texas Medicaid managed care: contact member services and review HHSC Ombudsman help.
- Government, other or unsure: identify the plan’s process before assuming an authority.
