Document an emergency-care denial.

Begin with the plan’s written reason and the facts you can support.

Start with the decision notice

Locate the coverage decision, benefit terms and appeal instructions. Record whether the stated reason concerns emergency need, coverage, network status or missing information. A bill alone may not explain why a claim was denied.

Describe what you understood at the time

Write a factual account of why emergency care was sought and what happened afterward. Keep identifying records in your own secure files. The builder can help organize a sample account, but it does not determine whether a legal standard was met.

Separate the complaint from the appeal

Check the notice for appeal deadlines and instructions. Filing a complaint does not automatically preserve an appeal deadline. Confirm the appropriate process for your plan before sending anything.
